Physical Form and Proof of Insurance Upload Tutorial

Step 1: Log into your Family Account and click on the "ADD" link under Physical Date column.

Step 2: Once you have uploaded your Physical Form you will be able to upload your Proof of Insurance if you were not able to at the time of registration.
